Community Access Group
Community Access Group (CAG) services are provided through a facilitybased program, where individuals attend a day program that engages them in meaningful activities tailored to their interests and preferences. The services offered encourage social networking, utilize natural sources of support, and promote increased independence, contribution, and inclusion in community life.
Community Access Individual
Community Access Individual (CAI) services are provided to individuals on a one-to-one basis, with one staff member assigned to each individual. The primary aim of CAI services is to enhance the individual's ability to engage with the community by developing skills and increasing natural supports.
CAI services are designed to be educational and supportive, helping individuals acquire, maintain, or improve their socialization and networking skills, as well as their ability to independently utilize community resources. These services also focus on developing adaptive skills necessary for active participation in the community outside of the individual's residence.
Also, CAI services are not facility-based.
Community Living Support
Community Living Support (CLS) Services are specifically designed to enhance life skills, allowing individuals to live independently in their own homes or with their families.
While personal care and assistance may be part of CLS services, the primary focus is on teaching skills related to activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
CLS services may include the following:
- Training and assistance with activities of daily living (ADLs), such as bathing, dressing, toileting, and transferring.
- Teaching and support in performing instrumental activities of daily living (IADLs), which may include personal hygiene, light housework, laundry, meal preparation, grocery shopping, using the telephone, and managing medication and finances.
Specialized Medical Supplies
Specialized Medical Supplies (SMS) Services provide a range of supplies designed to help individuals interact more independently with their environment. These supplies enhance quality of life and reduce reliance on physical support from others. SMS includes items such as nutritional supplements, specialized clothing, diapers, bedwetting protection pads, and other supplies specified in the approved Individual Service Plan and not available through other non-waiver Medicaid programs.
Respite In Home
Respite in Home Services offers temporary support or relief for family members or other unpaid caregivers of individuals with disabilities.
The most common reasons for utilizing this service include:
1. Families or unpaid caregivers needing support or relief to leave the home for part of the day.
2. The individual requiring a break from their caregiver.
3. Situations where relief from caregiving is necessary due to unavoidable circumstances, such as a family emergency.
